Thomasville, Georgia – August 1, 2025 – Vascular Interventional of Thomasville Associates (“VITA”) is announcing the closure of its practice at 900 Gordon Avenue, Thomasville, Ga 31792. VITA will close its practice on September 5, 2025.
Up and until September 5 th , 2025, any current or past patient of VITA, may obtain copies of his or her medical records by contacting VITA at 229-226-0125. A nominal copying fee may apply. Alternatively, up and until September 5 th , 2025, any current or past patient of VITA may request VITA to transfer his or medical records to another physician or provider of his or her choice by contacting VITA.
